LBBB

Cause

LBBB occurs when the electrical impulse from the AV node is delayed or interrupted in either the main left bundle branch or in both the anterior and posterior fascicles. The impulse continues down the right bundle branch normally then enters the left ventricle. This delay in ventricular depolarization accounts for the shape of the QRS on the ECG.

Symptoms

People with LBBB generally do not get symptoms unless it is severe enough to cause infranodal A-V block and slow heart rates. Dizziness may be felt.

Signs

Diagnostic Tests

ECG echo

Differential Diagnosis

Treatment

LBBB on its own is not generally treated. follow up tests are needed to rule out any underlying problems such as poor functioning left ventricle. an Echo is usually performed and a 24hr holter onitor to see if it is an intermittent conduction problem. A yearly ECG is recommended.
